New Patient Info
We welcome new patients to our office. Please arrive 15 minutes before your appointed time to complete the patient forms necessary to treat you safely and efficently. The attached file contains these forms. Printing them, filling them out and bringing them with you will allow us to meet your needs more quickly than completing them on your arrival. We know your time is important and we make every effort to see you at your appointed time. We see our patient's painful emergencies promptly to relieve their distress and occasionally this may affect appointment time.

For your convenience, we accept cash, personal checks, money orders and major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
Our business staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company, not our office. We have no influence with them. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your policy covers. You should have been given a copy of coverage from your human resources department, if not request it. You are the best advocate for your coverage.
The fees charged for services rendered are the same for all of our patients. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available and they sell a variety of policy types. Also, some companies take care of claims promptly while others delay payment indefinitely. When payment from your insurance company is delayed please contact your human resources department for help. They decide which insurer you get and pay the premium. They can get prompt resolution as they can seek a different insurance company at will.
