Registration and Health History Form
We accept cash, personal checks, money orders, and major credit cards for your convenience. Payment is expected at the time services are performed. When more extensive dental care is necessary, our office can make financial arrangements.
Our business staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company, not our office. We do not have any influence on them. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your policy covers. You should have been given a copy of coverage from your human resources department, if not request it. You are the best advocate for your coverage.
The fees charged for services rendered are the same for all of our patients. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available, and they sell a variety of policy types. Also, some companies take care of claims promptly while others delay payment indefinitely. When payment from your insurance company is delayed, please get in touch with your human resources department for help. They decide which insurer you get and pay the premium. They can get a prompt resolution as they can seek a different insurance company at will.
Thank you, and please call or email our office if you have any questions at all.
